The CARNA Continuing Competence Program (CCP) worksheets and forms are designed to help you complete and document all steps of the continuing competence process.

Documentation of your continuing competence activities is an official record of participation in the CCP and demonstrates that you have met the CCP requirements.

Your documentation must be clear, logical and chronological, and should be consistent with the information reported to CARNA at registration/renewal.

You are not required to use the CARNA CC worksheets. You can use forms of your own design or forms developed by your employer. You may choose to develop a portfolio. However, you must be able to provide written evidence, when asked, of having completed all steps of the CC process.

Your self-assessments, learning plans and any supporting documentation relevant to your CCP must be kept for five years as evidence of your activities.

Please note:

• Collecting Feedback is a core element of the CARNA Continuing Competence Program; however, you are not required to submit the feedback you received.
  
• The worksheets Reflecting on My Practice, Reflecting on My Nurse Practitioner Practice and Tracking Other Learning are supplemental tools for your use only and you are not required to submit these to CARNA.
